As to what went wrong, I would guess that the setup of the image grid ("Select Images" tab) is not correct. If you copy the gigapan file to a new file name, say z.gigapan, and open that, you can see the grid of little thumbnail images again. I would scan the 4x4 grid with your eyes carefully to see that each row and column looks like it's properly arranged.
